What is Robotic APR Surgery?

Abdominoperineal Resection (APR) is a surgery performed for cancers located very low in the rectum, where preservation of the anal sphincter is not possible.
In robotic APR, the surgery is performed using advanced robotic technology, allowing precise dissection deep inside the pelvis with better visualization and control.

This involves removal of:

Rectum

Anal canal

Surrounding lymph nodes

A permanent colostomy is created to pass stool.

When is APR Required?

APR is recommended in the following situations:

Very low rectal cancers (close to anal sphincter)

Tumours involving sphincter muscles

Residual disease after chemoradiotherapy

Recurrent rectal cancer

When sphincter preservation is oncologically unsafe

Decision is based on tumour location, MRI findings, and response to treatment.

Why Robotic APR Surgery?

Robotic technology offers significant advantages in pelvic cancer surgery:

Superior precision in narrow pelvic space

3D magnified vision for better tumour clearance

Better nerve preservation (important for urinary & sexual function)

Reduced blood loss

Lower conversion to open surgery

Faster recovery and early mobilisation

Especially beneficial in difficult pelvic anatomy.

How Robotic APR is Performed

Procedure
  • Robotic ports placed through small abdominal incisions
  • Precise dissection of rectum under 3D magnified vision
  • Total mesorectal excision (TME) performed
  • Perineal phase to remove anal canal
  • Permanent colostomy created

Ensures complete tumour removal with precision.

Recovery & Outcomes

Hospital stay: ~5–7 days

Less pain compared to open surgery

Faster recovery and mobilisation

Better wound healing

Improved quality of life with proper stoma care guidance

Structured rehabilitation and stoma education is key.

Safety & Results

Robotic APR is safe and oncologically effective when performed by experienced surgeons:

Adequate margins and lymph node clearance

Lower complication rates in expert centres

Better visualization reduces surgical errors

Comparable or improved outcomes vs open/laparoscopic surgery

Surgeon experience and case selection are critical.
